Abstract

Latar Belakang: Dalam beberapa kasus, infeksi dapat dikaitkan dengan respon tubuh yang tidak teratur disertai disfungsi organ, atau disebut sepsis. Salah satu disfungsi yan disebabkan oleh sepsis adalah disrupsi pada hemostatik. Pada saat infeksi, terjadi respon imun yang mengaktifkan jalur inflamasi dan koagulasi secara simultan. Karena itu, trombosit dan indeksnya sangat mungkin terpengaruh pada pasien sepsis. Sementara itu, untuk mengidentifikasi disfungsi organ pada pasien sepsis digunakan parameter skor SOFA. Penelitian ini bertujuan untuk mengetahui korelasi antara indeks trombosit dengan skor SOFA pasien infeksi di ICU RSUD Raden Mattaher Jambi. Metode: Penelitian ini adalah penelitian analitik yang melibatkan 36 responden, menggunakan teknik consecutive sampling. Penelitian dilakukan di ICU RSUD Raden Mattaher. Penelitian ini dilakukan dengan cara menghitung kadar indeks trombosit dan skor SOFA pada jam ke-0, 24, 72 dan 144. Hasil: Hasil penelitian terhadap 36 responden, didapatkan pasien dengan jenis kelamin laki-laki sebanyak 22 orang (61,1%) dan perempuan 14 (38.9%). Keluaran responden yang meninggal sebesar 13 orang (36.1%). Berdasarkan hasil analisis survival diagram Kaplan-Meier didapatkan tren peningkatan MPV, PDW, dan skor SOFA, serta penurunan pada PCT dan PLT pada kelompok yang meninggal. Berdasarkan hasil uji statistik pearson, didapatkan korelasi yang signifikan antara MPV dan PLT terhadap skor SOFA pada jam ke- 0, 24 dan 144, serta antara PCT terhadap skor SOFA pada jam ke- 0 dan 144 (p<0.05) Kesimpulan: Secara keseluruhan, didapatkan korelasi positif yang bermakna antara MPV dan skor SOFA, korelasi negatif yang signifikan antara PLT dan skor SOFA, dan korelasi negatif dengan hubungan tingkat sedang antara PCT dan skor SOFA, serta tidak ditemukan korelasi signifikan antara PDW dan skor SOFA pada pasien ICU RSUD Raden Mattaher Jambi. Perlu diteliti lebih lanjut untuk menemukan penyebab hal tersebut tidak berhubungan. Background: In some cases infection can be associated by irregular body responses accompanied with organ dysfunction, these responses were called as sepsis. One of dysfunction which sepsis caused is the disruption of hemostatic. At the time of infection, there is a series of immune responses which will activate inflammation and koagulation path silmutaniously. Therefore, platelet and its indices most likely will be affected on sepsis patient. Meanwhile, to identify organ dysfunction in sepsis patient can be assessed by using the SOFA scores parameter. The purpose of this study was to determine the relationship between Platelet indices level and SOFA score of infection patients in the Intensive Care Unit RSUD Raden Mattaher Jambi. Method: This study was an analytic study. Respondents are consisted of 36 people, and collected by using consecutive sampling. This research was conductedin ICU at Raden Mattaher Hospital, Jambi. This research was conducted by measuring the platetlet indices level and SOFA scores at 0, 24, 72 and 144 hours. Results: The result of this study from 36 respondents, showed that were 22 men (61.1%) and 14 women (38.9%). Based on respondent outcome, we found that 13 (36.1%) were died. Based on survival diagram of Kaplan Meier, MPV, PDW and SOFA score showed incresed trends, while PCT and PLT decreased in the non-survivor group. Pearson test showed that there was a statistically significant correlation between MPV and PLT levels to SOFA score at 0, 24 and 144 hour, while PCT to SOFA score at 0 and 144 hour (p<0.05) Conclusion: Overall, there was significant positive correlation between MPV and SOFA score, significant negative correlation between PLT and SOFA score, moderate negative correlation between PCT and SOFA score, and there was no significant correlation between PDW and SOFA score in ICU patient in RSUD Raden Mattaher Jambi. It is necessary to find the factors that cause it to be unrelated.
