Abstract

ABSTRAK Di negara seperti Indonesia yang jumlah pertumbuhan penduduknya meningkat pesat dan banyak usia lanjut, angka penyakit degeneratif yang semakin meningkat dan kondisi demografi yang terdiri dari pulau-pulau maka konsep home care sangat cocok digunakan. Konsep home care ini merupakan solusi paling tepat untuk mengantisipasi jumlah pasien yang tidak tertampung di rumah sakit. Hasil observasi dan wawancara penulis dengan coordinator dan tim home care di Puskesmas Kenali Besar Kota Jambi didapatkan bahwa program home care Puskesmas Kenali Besar tidak memiliki Kerangka Acuan Kegiatan dan Rencana Usulan Kegiatan yang terbaru, belum adanya SOP home care yang optimal dan belum adanya format asuhan keperawatan home care yang optimal. Beberapa kendala tersebut menyebabkan tidak optimalnya program home care yang ada di Puskesmas Kenali Besar sehingga nantinya berdampak pada kualitas pelayanan pada masyarakat. Alternatif pemecahan masalahnya adalah membantu adalam menyusun kelengkapan administrasi pada manajemen program home care. Intervensi dilakukan selama dua hari dengan membantu menyusun kelengkapan administrasi Bersama koordinator program dan tim. Hasil dari intervensi adalah, koordinator dan tim program home care mamahami dan lebih optimal dalam menyusun dan membuat administrasi kelengkapan program home care. Kata Kunci: Manajemen, Program Home Care, Evaluasi, Puskesmas ABSTRACT In a country like Indonesia, where the number of population growth is increasing rapidly and there are many elderly people, the number of degenerative diseases is increasing and the demographic condition which consists of islands, the concept of home care is very suitable to be used. This home care concept is the most appropriate solution to anticipate the number of patients who are not accommodated in the hospital. The results of the author's observations and interviews with the coordinator and home care team at the Kenali Besar Health Center in Jambi City found that the Kenali Besar Health Center home care program does not have the latest Activity Terms of Reference and Activity Proposed Plans, there is no optimal SOP for home care and there is no nursing care format. optimal home care. Some of these obstacles caused the home care program at the Kenali Besar Health Center to be not optimal so that later it had an impact on the quality of service to the community. An alternative solution to the problem is to assist in compiling administrative completeness in the management of the home care program. The intervention was carried out for two days by helping to arrange administrative completeness with the program coordinator and the team. The result of the intervention was that the coordinator and team of the home care program understood and were more optimal in preparing and administering the completeness of the home care program.
