Abstract

ABSTRAK Latar Belakang : Discharge planning merupakan proses berkesinambungan guna menyiapkan perawatan mandiri pasienpasca rawat inap. Discharge planning saat ini dianggap sebagai bagian penting dari layanan kesehatan karena merupakan suatu proses berkesinambungan yang harus diterapkan sejak pasien masuk dan dirawat inap di rumah sakit hingga evaluasi tindakan pada saat pasien akan pulang untuk mengkaji kemungkinan rujukan atau perawatan lanjut di rumah sesuai kebutuhan agar dapat membantu pasien dan keluarga untuk meningkatkan serta mempertahankan derajat kesehatannya.Tujuan studi kasusadalah mengidentifikasi gambaran pengetahuan perawat tentang discharge planning di ruang Lambda RSJD Provinsi Jambi. Metode : Studi kasus menggunakan kuesioner pengetahuan perawat tentang discharge planning di ruang Lambda RSJD Provinsi Jambi. Hasil : Berdasarkan hasil studi kasus mengenai gambaran pengetahuan perawat tentang discharge planning di ruang Lambda RSJD Provinsi Jambi didapatkan sebagian besar perawat di ruang Lambda memiliki pengetahuan cukup mengenai konsep teori discharge planning yaitu 57%. yang memiliki pengetahuan baik mengenai konsep teori discharge planning sebanyak 7% dan yang memiliki pengetahuan kurang mengenai konsep teori discharge planning sebanyak 36%. Saran bagi pihak rumah sakit agar meningkatkan pengetahuan perawat dengan memberikan sosialisasi, pelatihan, pengawasan dan pendampingan dalam pelaksanaan discharge planning Kata kunci : Pengetahuan, discharge planning, Perawat ABSTRACT Background: Discharge planning is a continuous process to prepare post-hospitalized patient self-care. Discharge planning is currently considered an important part of health services because it is a continuous process that must be implemented from the time the patient enters and is admitted to the hospital until the evaluation of the action when the patient is going home to examine possible referrals or further care at home as needed in order to help patients and families to improve and maintain their health status. The purpose of the case study was to identify an overview of nurses' knowledge about discharge planning in the Lambda room of the Jambi Provincial Hospital. Methods: The case study used a nurse's knowledge questionnaire about discharge planning in the Lambda Room RSJD Jambi Province. Results: Based on the results of a case study regarding the description of nurses' knowledge about discharge planning in the Lambda room of the Jambi Provincial Hospital, it was found that most of the nurses in the Lambda room had sufficient knowledge of the concept of discharge planning theory, namely 57%. those who have good knowledge about the concept of discharge planning theory are 7% and those who have less knowledge about the concept of discharge planning theory are 36%. Suggestions for the hospital to increase the knowledge of nurses by providing socialization, training, supervision and assistance in implementing discharge planning Keywords: Knowledge, discharge planning, Nurse
