Abstract
Background: Leptospirosis is a zoonotic disease that frequently affects farmers due to contact with environments contaminated by Leptospira bacteria. Environmental risk factors such as physical living conditions and climate play a significant role in disease transmission, especially in tropical areas with poor sanitation. Methods: This study employed a systematic literature review approach by analyzing 9 research articles related to environmental risk factors influencing leptospirosis incidence among farmers. The process involved data searching, article screening, information extraction, and narrative synthesis to assess risk factors from existing literature. Results: The review found that high housing density, poor environmental cleanliness, presence of rodents as Leptospira reservoirs, dense vegetation, lowland topography, and flooding were consistently associated with increased leptospirosis risk. Weather factors such as high rainfall, warm temperature, and high humidity also enhanced bacterial survival and infection risk. Besides environmental factors, farmers' behaviors in maintaining hygiene and using personal protective equipment influenced disease occurrence. Conclusion: Physical environmental and climatic factors significantly affect leptospirosis occurrence among farmers. The combination of poor environmental conditions and insufficient preventive actions by farmers is a primary determinant of infection risk. Prevention efforts should focus on improving environmental sanitation, controlling rodent populations, and educating farmers on protective measures.
